The Twisted Nose

My nose is crooked, what do you do during rhinoplasty surgery to fix it?

Answer:
The twisted nose is one of the more technically challenging problems in rhinoplasty, requiring an experienced and skillful rhinoplasty surgeon. In the hands of an experienced and skillful surgeon, significant improvement is usually possible. Sometimes the nose can be made completely straight; however, the patient should expect some deviation to persist.

The techniques used in each case depend upon the reasons for the twisting of the nose. The nose can be thought of as being divided into an upper third, middle third and lower third (tip). The upper third is essentially the bony portion of the nose, the middle third is essentially the cartilaginous bridge of the nose, and the lower third is the tip of the nose.

Each part of the nose must be carefully analyzed by the surgeon for any deviation. Depending upon the nature of the deviation in each part of the nose, different techniques will be selected. A "game plan" is put together for the entire nose that takes into consideration the interaction of all the "parts." Thus, a simple plan is undertaken for a simple deviation, while a complex deviation may require a complex surgical plan.

If the nose is twisted because of the upper, bony portion of the nose, Dr. Becker will need to "break" the nasal bones and re-orient them in a straighter position.

If the nose is twisted because of a twist in the tip, Dr. Becker would perform "tip-plasty" and use a variety of maneuvers to make the nasal tip cartilages and the nasal tip sit straighter.

And, if the middle portion of the nose - the cartilaginous bridge - is twisted, Dr. Becker will use a variety of techniques in a graduated, step-wise approach to straighten this region.

Why did my nose get twisted like this?

To fix a twisted nose, do you remove tissue from one side, add tissue to the other, or do both?

My nose is all twisted up, and as a result, my nostrils are uneven, will this get fixed too?

I'm not sure my nose is twisted, it almost looks like there's a bump on one side that makes it look that way. Do you still do the same thing to fix it?

Instead of the top of my nose being twisted, the bottom front part is twisted. Can you fix this?
